ABSTRACT

Anti-H. pylori urease IgY: effective mechanism and application in controlling H. pylori infection in human.

Helicobacter pylori (H. pylori) is considered the most important cause of gastritis, gastric ulcer and stomach cancer. Recent investigations revealed that about 50% population in the world is infected with H. pylori. Medication with a combination of 2 antibiotics and PPI is the common therapy of H. pylori infection. But antibiotic therapy fails in 10-20% of cases due to the development of antibiotic resistance.

While most microbes are killed in the acidic environment in the stomach, H. pylori can survive and grow there because it produces a strong urease enzyme that accumulates on the surface of the bacterium. The enzyme degrades urea in foods to form ammonium and carbonic acid. This reaction increases local pH in stomach enabling the bacterium to grow. Besides, urease also functions as an adhesin to allow H. pylori to bind firmly on gastric mucin layer.

In our study, we extracted urease from H. pylori and used it as an antigen to immunize layer chickens. From the eggs laid by the immunized chickens we prepared specific antibody (IgY) and examined its efficacy against H. pylori on human volunteers. Various clinical trials conducted in Japan, Korea, Taiwan and Vietnam show that the IgY significantly decreased H. pylori number in the stomach as well as reduced gastritis symptoms in patients. When combined with antibiotics and PPI the IgY substantially enhanced the therapeutic efficacy of the drugs especially in patients having antibiotic-resistant H. pylori. The mechanism behind this synergetic effect will be explained.

Evaluation of a health food supplement containing anti-H. pylori urease IgY antibody on patients with chronic gastritis in Hanoi, Vietnam

[Background] Helicobacter pylori is considered the leading cause of gastritis, gastric ulcer and stomach cancer. Medication with a combination of 2-3 antibiotics is the common therapy of H. pylori infection. But antibiotic therapy fails in 10-20% of cases due to the development of antibiotic resistance. Studies conducted by IRIG (Gifu, Japan) in the past 10 years show that egg antibody (IgY) against urease of H. pylori is effective in reducing the bacterial load in stomach of H. pylori positive volunteers. This study was conducted to examine if combination of the antibody with current treatment regime could improve the eradication rate and clinical signs in H. pylori positive gastritis patients.

[Methods] Volunteers were recruited among patients visiting 108 hospital. The volunteers that passed the selection criteria were divided into a test and a control group. Both groups were treated with a current treatment regime but only the test group received IgY-containing food supplement (GastimunHP, EW Nutrition Japan) as an adjunctive measure for 15 days. The subjects were examined before and 4 weeks after the treatment initiation by UBT and gastro-endoscopy.

[Results] 77 patients who were positive with H. pylori and had gastritis or gastro-duodenal ulcer were divided randomly into test (n = 36) and control (n = 41) groups. The average UBT levels in the test group at baseline and 4 weeks posttreatment were 161.64 ± 83.53 and 49.42 ± 44.21, respectively. There was a significant decrease in UBT in this group after the treatment (p < 0.0001). 28 patients in this group became H. pylori negative by UBT test (77.78%). The UBT levels for the control group were 158.54 ± 43.73 at baseline and 73.37 ± 30.0 at 4 weeks, respectively, and the difference was statistically significant (p < 0.0001). 17 patients in this group became H. pylori negative by UBT test (41.46%). Mean changes of UBT in the test and control group were 112.22 ± 43.73 and 85.17 ± 37.9, respectively, and the difference between the 2 groups was statistically significant (P = 0.002). Clinical symptoms including stomach pains, nausea, bloating, belching, and vomiting had a tendency to improve faster in the test group compared to the control group.

[Conclusions] Food supplement fortified with IgY against urease of H. pylori enhances eradication of H. pylori when used in combination with medicines. This antibody could be useful especially in case of antibiotic-resistant H.pylori infections.

BACKGROUND
Helicobacter pylori (H. pylori) is one of the most common chronic bacterial infections in humans, affecting half of the world’s population. Infected people may develop gastritis, gastric ulcer or even gastric cancer. Current therapy regimes use a combination of 2-3 antibacterial medicines and a proton pump inhibitor but fail in 10-20% of cases due to the increasing drug resistance. Oral administration of egg yolk immunoglobulinY (IgY) has been reported as a means of effective immunotherapy for H. pylori infections, but its protective mechanism in stomach needs to be clarified. In this study, we investigated the in vitro resistance of purified IgY against pepsin in low pH medium and its suppressive effect against H. pylori in a challenged mouse model.

METHODS
Recombinant urease antigens were prepared and used to immunize chicken hens to generate anti-HP urease IgY. The purified IgY was mixed with simulated gastric fluid (SGF, pH 4.1) containing pepsin and then incubated at 37°C for 10 h with constant shaking. Samples were collected every hour and the residual antibody activity was measured by enzyme-linked immunosorbent assay (ELISA). NS:Hr/ICR hairless mice were infected with H. pylori, treated with various doses of anti-HP IgY and the number of the bacterium in stomach was assessed.

 RESULTS
Anti-HP IgY showed strong resistance against pepsin during SGF digestion treatment retaining intact structure up to 9 hours. In the same condition mammalian IgG showed lower pepsin resistance compared to IgY completely disappearing in SDS-PAGE after 9 h of digestion. Anti-HP IgY significantly reduced the number of H. pylori in NS:Hr/ICR hairless mice challenged with H. pylori and improved inflammation level in gastric mucosa.

 CONCLUSIONS
Chicken egg yolk IgY is highly resistant to pepsin in stomach and shows good suppression against H. pylori in a mouse challenge model. Considering the various advantages of IgY, the antibody could be useful as a novel therapeutic and prophylactic agent for controlling H. pylori infections in humans.

 Background and Objective: Egg yolk antibody (IgY) against gingipains from Porphyromonas gingivalis is known to have good effects when applied directly in periodontal pockets after subgingival scaling and root planing (SRP) treatment in patients with chronic periodontitis. But subgingival application of IgY can be done only by dentists. The aim of the present study was to examine the clinical and bacteriological effects of daily use of lozenges fortified with the IgY as an adjunct to nonsurgical therapy.

 Materials and Methods: Sixty four patients with chronic periodontitis were recruited by convenience sampling method and divided randomly into a test and a placebo group. The two groups were treated by SRP followed by a daily use of lozenges containing either specific IgY against P. gingivalis gingipains (test) or a sham-immune IgY (placebo). Clinical parameters including bleeding on probing (BOP), probing pocket depth (PD) and quantitative analysis of P. gingivalis in the gingival pockets were assessed at baseline and 8 weeks after the initiation of treatment.

 Results: There was no statistically significant difference in the examined parameters at baseline between the two groups. Both groups showed significant improvement of all parameters at 8 weeks post treatment. There was a significant difference in improvement of BOP and P. gingivalis level in the test group compared to the placebo group. The reduction of PD was bigger in the test group compared to the placebo group although there was no significant difference between the 2 groups on this parameter at 8 weeks post treatment.

 Conclusion: Lozenges containing IgY antibody against gingipains from P. gingivalis could be a useful adjunct to nonsurgical therapy in chronic periodontitis.
